HIPAA Notice of Privacy Practices (NPP)

As a Licensed Clinical Social Worker, I am a covered entity under HIPAA. This means your health information is legally protected.

• Your records will not be shared without your written permission, except in cases required by law (such as risk of harm to self/others, child/elder abuse, or court order).

• You have the right to request a copy of your records, ask for corrections, and know who your information has been shared with.

• Electronic communications are secured, but no system is 100% risk-free. Secure telehealth platforms and encrypted email are used to protect your information.
